Telehealth Consent
Review this consent before using telehealth services coordinated through the Eternum platform.
Nature of telehealth and your provider
Telehealth allows you to receive healthcare remotely using an asynchronous questionnaire, secure messaging, and video when required by state law or clinical circumstances. Clinical services are provided by licensed providers of Arora Health & Aesthetics, LLC.
Services are available only in jurisdictions where affiliated medical providers are licensed to practice and where pharmacy partners are authorized to dispense medications. Availability may vary by state and is subject to applicable laws, regulations, and licensure requirements.
Benefits, risks, and limitations
Potential benefits include convenient access to licensed clinicians and continuity through secure communication. Risks include technology or connection failures, delays, incomplete information, limits on physical examination, and privacy or security risks.
A provider may require additional information, video, testing, an in-person examination, or referral. Telehealth may not be suitable for every condition. Participation does not guarantee a diagnosis, treatment, prescription, or particular outcome.
Your choices and responsibilities
You have the right to refuse telehealth or withdraw consent. Tell your provider through secure messaging if you wish to withdraw; you may seek in-person care or another provider. Withdrawal does not undo care already provided or eliminate applicable charges.
Provide accurate identity, location, medical history, current medication, allergy, and other health information. Update your provider when circumstances change, follow treatment and monitoring instructions, and promptly report side effects.
Telehealth is not for emergencies. Call 911 or seek emergency care if you have a medical emergency rather than waiting for a portal or messaging response.
Prescriptions and compounded medications
Prescriptions are issued only when clinically appropriate and may be filled by a licensed 503A compounding pharmacy. A prescription is not guaranteed.
Compounded medications are not FDA-approved and have not been evaluated by the FDA for safety, effectiveness, or quality. Discuss risks, benefits, approved alternatives, and formulation-specific instructions with your clinician and pharmacist.
Records and privacy
Health records and communications are handled as described in the HIPAA Privacy Policy. Telehealth has privacy risks despite security safeguards. Use a private location and secure device for your encounters and account access.
Consent acknowledgement
By acknowledging this consent during intake or before receiving telehealth services, I confirm that I have read and understand the nature, benefits, risks, and limitations of telehealth; understand that a prescription is not guaranteed; have had the opportunity to ask questions; and voluntarily consent to receive telehealth care. I understand my right to refuse or withdraw consent.
Reading this page alone does not record your consent. The acknowledgement is provided through the intake or care process.
For questions about this consent or how to contact your provider, email hello@eternumrx.com or call (541) 871-3683. Do not send sensitive medical information by ordinary email.
